SEMI TRUCK ORDERS WITH SHORTAGE AND-OR DAMAGED MERCHANDISE

BEFORE YOU SIGN THIS FREIGHT BILL, MAKE SURE THERE ARE:_____________PIECES ON THIS PALLET! IN THE EVENT OF A “SHORTAGE”: PLEASE FOLLOWING THESE IMPORTANT GUIDELINES (1) ACCEPT THE SHIPMENT. (2) WRITE DOWN HOW MANY PIECES WERE RECEIVED (3) WRITE DOWN HOW MANY PIECES WERE SHORTED (4) THE TRUCK DRIVER WILL HAVE TO CALL INTO DISPTACH TO REPORT THE SHORTAGE. (5) IF THESE STEPS ARE FOLLOWED, WE WILL REPLACE THE SHORTAGE. (6) IF YOU SIGN THE FREIGHT DELIVER RECEIPT WITH NO EXCEPTIONS NOTED, YOU ARE STATING THAT THE MERCHANDISE WAS DELIVERED WITH NO SHORTAGE AND THERE IS NOTHING WRONG WITH THIS ORDER. YOU WILL NOT BE REIMBURSED FOR THE MERCHADISE THAT IS MISSING. BEFORE YOU SIGN THE FREIGHT BILL, MAKE SURE THERE IS NO DAMAGE! IN THE EVENT OF DAMAGED MERCHANDISE, PLEASE FOLLOW THESE VERY IMPORTANT GUIDELINES: (1) ACCEPT THE ENTIRE FREIGHT/LTL SHIPMENT. (2) CALL USLC TOLL FREE IMMEDIATELY @ (800) 325-4294 WHILE THE DRIVER IS THERE. (3) WRITE DOWN THE EXACT DAMAGED THAT HAS TAKEN PLACE ON THE DELIVERY BILL. (4) HOW MANY ITEMS WERE DAMAGED. (5) TAKE PICTURES OF THE DAMAGED MERCHANDISE. DO NOT THROW AWAY ANYTHING!!!!!!!!!!!! UNTIL WE NOTIFY YOU TO DO SO. (6) EMAIL USLC PICTURES: SALES@USLIGHTINGCORP.COM. (7) THE TRUCK DRIVER WILL HAVE TO CALL INTO DISPATCH TO REPORT THE DAMAGE. (8) RETAIN THE DAMAGED GOODS FOR POSSIBLE ON SITE INPSECTION. THIS COULD TAKE UP TO 10 WEEKS. (9) IF THESE STEPS ARE FOLLOWED, WE WILL SHIP OUT THE REPLACEMENT MERCANDISE AND WE WILL FILE THE FREIGHT CLAIM WITH THE LTL CARRIER. (10) IF YOU SIGN THE FREIGHT BILL DELIVERY RECEIPT WITH NO DAMAGE NOTED, YOU ARE STATING THAT THERE IS NOTHING WRONG WITH THIS ORDER & YOU WILL NOT BE REIMBRUSED FOR THE DAMAGED MERCHANDISE.
